cze Porovnání hladin celkového homocysteinu v plazmě mezi pacienty s pozitivním a negativním koronarografickým nálezem Několik klinických studiích naznačilo, že vysoká hladina homocysteinu v plazmě je nezávislým rizikovým faktorem aterosklerózy. Stupeň aterosklerotických změn věnčitých cév mohou být zjištěny tzv. koronarografií. Cílem této studie bylo porovnat hladiny celkového homocysteinu u dvou skupin pacientů s rozdílnými koronarografickými nálezy. První skupinou bylo 50 pacientů (33 mužů a 17 žen) s pozitivním nálezem. Průměrný věk pacientů byl 65,5 ? 13,9 let. Pozitivní koronarografie byla charakterizována jako 50-ti a více procentní stenóza alespoň v jedné z věnčitých cév. V druhé skupině pacientů s negativní koronarografií bylo 46 pacientů (24 mužů a 22 žen). Průměrný věk pacientů byl 61,5 ? 13,9 let. Negativní koronarografie byla charakterizována jako žádná stenóza ve věnčitých cévách. Celkový homocystein v plazmě jsme stanovovali pomocí ionpárové RP-HPLC s coulometrickou elektrochemickou detekcí. Tato metoda je levná s rychlou přípravou vzorku a s dostatečnými analytickými parametry. Tato HPLC metoda s elektrochemickou detekcí byla porovnána s komerčně dostupnou imunochemickou metodou Immulite. Rovnice lineární regrese byla následující: HPLC/ED = 1,0679 (Immulite) + 0,8485 umol/l, R2 = 0,9925. Pro každé měření jsme používali komerční kontrolní plazmu. Hladiny celkového homocysteinu v plazmě byly u pacientů s pozitivní koronarografií signifikantně vyšší než u pacientů s negativním koronarografickým nálezem (p<0,05). homocystein; atherosclerosa
eng Comparison of levels of total plasma homocysteine in patients with positive coronarography and patients with negative coronarography Several clinical studies have indicated that high plasma homocysteine concentration is an independent risk factor for origin and progress of atherosclerosis. The grade of atherosclerotic changes in coronary arteries may be investigated with coronarography. The aim of study was to compare levels of total plasma homocysteine in two groups of patients with different coronarography findings. There were 50 patients (33 men and 17 women) with positive findings in the first group. The mean value of age was 65.5 ? 13.9. Positive coronarography was characterized as 50% or more stenosis at least in one of the epicardial coronary arteries. There were 46 patients (24 men and 22 women) with negative findings in the second group. The mean value of age was 61.5 ? 13.9. Negative coronarography was characterized as no stenosis at least in one of the epicardial coronary arteries. We have determined concentration of total plasma homocysteine using ion-paired reversed-phase HPLC with coulometric electrochemical detection. This method is an inexpensive with rapid sample preparation and showed satisfactory analytical sensitivity, precision and linearity. HPLC/ECD method was compared with commercially available immunochemical assay Immulite by linear regression: HPLC/ED = 1.0679 (Immulite) + 0,8485 umol/l, R2 = 0,9925. We had used control samples before every set of measuring.
