cze	Změna profilu sulfátů a sfingomyelinů v plazmě, moči a tkáních u pacientů s karcinomem ledvinových buněk	Renální karcinom ledvin (RCC) je nejběžnějším typem rakoviny ledvin a je spojován s vysokou mortalitou. V tělních tekutinách nebyl doposud objeven žádný specifický biomarker pro RCC v tělních tekutinách, takže stále neexistuje žádný neinvazivní diagnostický test. Nedávno jsme popsali významně změněný profil sulfatidů v RCC nádorových tkáních, což nás motivovalo zjistit, zdali jsou tyto alterace specifických lipidů reflektovány i ve sbíraných tělních tekutinách pacientů, a zdali umožňují detekci RCC. Metoda: Pro tento účel jsme nasbírali a následně analyzovali 143 vzorků plazmy, 100 vzorků moči a 154 vzorků tkání od 155 pacientů s rakovinou a společně s tím jsme nasbírali dále 207 vzorků plazmy, 70 vzorků moči od 214 zdravých kontrol. Výsledky: V této práci poprvé popisujeme zvýšené koncentrace laktosylsulfatidů a snížené hladiny sulfatidů s hydroxylovanými mastnými acyly v tělních tekutinách pacientů s RCC ve srovnání s kontrolami. Tyto změny jsou vyšší u pacientů s pokročilým stádiem nádoru. Použité klasifikační modely dokáží rozlišit pacienty s RCC od kontrol. V případě všech měřených vzorků plazmy byla AUC pro testovací sadu 0,903 (0,844-0,954), zatímco u vzorků moči 0,867 (0,763-0,953). Na základě vzorků plazmy jsou modely schopny efektivně detekovat i pacienty s časným a pozdním stadiem RCC. Citlivost testovacího setu byla 80,6 % a 90 % a hodnoty AUC byly 0,899 (0,832–0,952) pro pacienty s časným stádiem, resp. 0,981 (0,956–0,998) pro pacienty s pokročilým stádiem rakoviny. Závěr: Podobné trendy v tělních tekutinách a tkáních naznačují, že RCC ovlivňuje metabolismus lipidů, a zdůrazňují potenciál studovaných lipidů pro minimálně invazivní detekci rakoviny, včetně pacientů s ranými stadii nádoru, jak dokazuje prediktivní schopnost aplikovaných klasifikačních modelů.	biomarkery rakoviny; sulfatid; sfingomyeliny; lipidomika; klasifikační modely; včasná detekce; karcinom ledvinových buněk

A non-invasive diagnostic test remains unavailable due to the lack of RCC-specific biomarkers in body fluids. We have previously described a significantly altered profile of sulfatides in RCC tumor tissues, motivating us to investigate whether these alterations are reflected in collectible body fluids and whether they can enable RCC detection. Methods: We collected and further analyzed 143 plasma, 100 urine, and 154 tissue samples from 155 kidney cancer patients, together with 207 plasma and 70 urine samples from 214 healthy controls. Results: For the first time, we show elevated concentrations of lactosylsulfatides and decreased levels of sulfatides with hydroxylated fatty acyls in body fluids of RCC patients compared to controls. These alterations are emphasized in patients with the advanced tumor stage. Classification models are able to distinguish between controls and patients with RCC. In the case of all plasma samples, the AUC for the testing set was 0.903 (0.844–0.954), while for urine samples it was 0.867 (0.763–0.953). The models are able to efficiently detect patients with early and late-stage RCC based on plasma samples as well. The test set sensitivities were 80.6% and 90%, and AUC values were 0.899 (0.832–0.952) and 0.981 (0.956–0.998), respectively. Conclusion: Similar trends in body fluids and tissues indicate that RCC influences lipid metabolism, and highlight the potential of the studied lipids for minimally-invasive cancer detection, including patients with early tumor stages, as demonstrated by the predictive ability of the applied classification models.	cancer biomarkers; sulfatide; sphingomyelins; lipidomics; classification models; early detection; renal cell carcinoma
